Delivering Integrated health and care centres across Wales
Local Partnerships has helped the Welsh Government achieve its Programme for Government commitment of delivering integrated health and care centres across Wales to drive closer integration of health and care on the ground.
Our team drafted and developed business cases according to Welsh Government Better Business Case guidelines. With colleagues in Welsh Government and Welsh local authorities they went on to design an innovative funding stream to supplement existing Welsh Government funding streams.
The programme will deliver at least 50 integrated health and care hubs across Wales facilitating people remaining active and independent in their own homes and living longer, healthier and happier lives, so reducing the need for hospital care.
It will also help to deliver a future-proofed, whole system operating model for a more accessible and integrated approach to health and social care services in the community, and deliver services closer to home, designed around people’s needs.
Outcomes
The programme is now up and running, new integrated health and care facilities are being developed across Wales.
